Open Letter to all parents and athletes:

Dear Parents and Athletes:

     We are excited to have athletes back on campus.  We are now in Phase 2 (July 20th-August 14th)  as indicated by the National Federation of High School Sports, the Indiana High School Athletic Association, and the Indiana-In Class Guidelines.

     Beginning July 20th, all sports will be allowed to meet following the protocols set forth by state and national guidelines and the Pendleton Heights Return to Play Plan. Not all sports have practices scheduled during this phase.  Please refer to our athletic website for information on sport specific practice schedules or contact your athletes coach.

Covid-19 Facts

     The virus that causes Covid-19 can affect people of all ages.  Research from the Centers for Disease Control, and among others, has found that while children do get infected by Covid-19, relatively few children with Covid-19 are hospitalized.  However, some severe outcomes have been reported in children, and a child with a mild or even asymptomatic case of Covid-19 can spread the infection to others who may be far more vulnerable.  While it is not possible to eliminate all risks of furthering the spread of Covid-19, the current science suggests there are many steps schools can take to reduce risks to athletes, coaches, staff and their families.

     From July 20th-August 14th our coaches will receive Covid-19 Education Training.  Our coaches will be required to wear a face mask while working with your student athlete on campus.  Our coaches will also be asking each athlete about their health each time they are on campus.  We are asking athletes and staff to pre-screen for Covid-19 symptoms before arrival on campus. Any individual (staff or student) who experiences any symptoms listed below will not be allowed to take part in workouts and should contact his or her primary care provider or other appropriate health care professional.  

Self-Screen Symptoms:

If your child is experiencing any of the symptoms listed below, Do Not send them to any on campus activity.

*Cough     *Chills     *Muscle Pain     *Shortness of breath or difficulty breathing

*Repeated Shaking with Chills     *Headache     *Sore Throat    * Loss of taste or smell

*Diarrhea     *Feeling Feverish or a measured temperature 100 degrees or higher

*Known Close Contact with someone who is Lab Confirmed to have Covid-19

Student Athletes:

*All summer activities are voluntary.

*Student athletes must be registered in Final Forms prior to their first workout.

*Individuals are limited to 15 hours of activity per week.

*No sport may have more than two activity days per week.

*No sport may have activity on consecutive days.

*Students athletes will not be permitted on campus more than 3 hours per day.

*No physical contact is permitted during this phase.

*Wearing a face mask when possible is recommended.

*Student athletes must wash their hands prior to arrival and immediately following workouts.

*Students will not have access to the locker rooms and should come dressed appropriately.

*Sharing of clothing will not be permitted.

*Restrooms will be open, but social distancing will need to be practiced.

*Student athletes will be responsible for their own form of hydration.  Water fountain usage will not be permitted.  Please mark the athlete’s water bottle for personal identification.  Students may not share water bottles.

*Student athletes will only be permitted on campus during their designated activity time.

*Student athletes should use the provided hand sanitizer when entering the building.

  During Phase 2 (July 20th-August 14th) fall, winter and spring sports may start meeting.

IHSAA Guidance Regarding Pre-Participation Physical Examinations and Consent Forms for the 2020-21 School Year Due to the COVID-19 pandemic                               The Indiana High School Athletic Association, Inc. will waive the provisions of Rule 3-10 for the 2020-21 school year. Effective immediately, valid physical and consent forms presented to member schools on behalf of eligible students during the 2019-20 school year shall be valid during the upcoming 2020-21 school year. Transfer students moving from one member school to another member school must provide a copy of their valid 2019-2020 Pre-Participation Physical and Consent Form to the Receiving School in order to be eligible under Rule 3-8. Incoming students with a valid IHSAA Pre-Participation Physical and Consent Form completed and signed after April 1, 2019 will also be eligible to compete during the 2020-2021 athletic season.
Any athlete not having a valid IHSAA pre-participation examination physical form on file from the 2019-2020 scholastic year is required to have a valid Pre-Participation Physical and Consent Form completed prior to eligibility and competition.

If you choose to use a physical previously submitted for the 2019 school year, you will be required to sign a waiver stating that you knowingly and freely assume the risks and accept sole responsibility for the decision to allow my child to participate in IHSAA sports in the 2020-21 school year without an updated Pre-Participation Physical Examination and Consent Form. The waiver is located in Final Forms and must be signed.

An IHSAA Physical is REQUIRED to be on file with the PHMS Athletic Office prior to participation in ANY Athletic event.  This includes Conditioning, Open Field, Open Gym, Try-Outs and Practice.

All Required Forms and Documents must be complete and on file with the PHMS Athletic Office for participation in practice for all teams.

PHMS Athletics will be utilizing an online Form submission format beginning 20-21.  NOW is the time to work on the process for YOUR student.  PLease see the information below!!!


Parent registration 

How do I sign up? 

1. Go to: https://southmadison-in.finalforms.com/ 

2. Locate the parent icon and click NEW ACCOUNT below. 

3. Type your NAME, DATE OF BIRTH, and EMAIL. Next, click REGISTER. 

NOTE: You will receive an email within 2 minutes prompting you to confirm and complete your registration. If you do not receive an email, then check your spam folder. If you still can not locate the FinalForms email, then email support@finalforms.com informing our team of the issue. 

4. Check your email for an ACCOUNT CONFIRMATION EMAIL from the FinalForms Mailman. 

NOTE:  Once received and opened, click CONFIRM YOUR ACCOUNT in the email text.

5. Create your new FinalForms password. Next, click CONFIRM ACCOUNT. 

6. Click REGISTER STUDENT for your first child.

7. When all forms are complete, you will see a ‘Forms Finished’ message. 

IMPORTANT: If required by your district, an email will automatically be sent to the email address that you provided for your student that will prompt form your student to sign the required forms. 

How do I register additional students? 

Click MY STUDENTS. Then, repeat steps number 3 through number 7 for each additional student. 

How do I update information? 

Login at any time and click UPDATE FORMS to update information for any student. 

Registering a student 

What information will I need? 

Basic medical history and health information. Insurance company and policy number. Doctor, dentist, and medical specialist contact information. Hospital preference and contact information. 

How do I register my first student? 

IMPORTANT: If you followed the steps above you may Jump to Step number 3. 

1. Go to: https://southmadison-in.finalforms.com/ 

2. Click LOGIN under the Parent Icon. 

3. Locate and click the ADD STUDENT button. 

4. Type in the LEGAL NAME and other required information. Then, click CREATE STUDENT. 

5. If your student plans to participate in a sport, activity, or club, then click the checkbox for each. Then, click UPDATE after making your selection. Selections may be changed until the registration deadline. 

6. Complete each form and sign your full name (i.e. ‘Jonathan Smith’) in the parent signature field on each page. After signing each, click SUBMIT FORM and move on to the next form.
